Care Coordination
Collaborative support across your entire care team

What We Offer:
-
Comprehensive care coordination with your full treatment team—including case managers, physicians, physical therapists, occupational therapists, speech-language pathologists, rehabilitation psychologists, neuropsychologists, education teams, recreation therapists, and other specialists.
-
Communication bridge between you and your providers—helping you articulate your needs, experiences, and challenges to your care team in ways they can understand and act on.
-
Two-way feedback system where we learn from your providers how to best support their interventions, while educating them on the psychological and behavioral aspects of brain injury that impact treatment.
-
Development of communication models and accessibility systems tailored to your cognitive needs, reducing overwhelm and the administrative burden on you and your family.
-
Advocacy support to ensure your voice is heard, your needs are met, and your care plan truly works for your brain and your life.
-
Resource navigation and specialist referrals—connecting you with brain injury-specialized interventionists, community resources, support services, and programs that understand neurological differences.
-
Reduced workload for survivors and families by serving as a central point of contact, translating between providers, and managing the complexity of multi-disciplinary care.
Why Brain Injury-Informed Care Coordination Matters:
Navigating multiple providers, appointments, and treatment recommendations is exhausting for anyone—but especially overwhelming when your brain is already working harder to process information, remember details, and communicate needs. Coordinated care isn't just convenient; it's essential for recovery. When your providers communicate with each other and understand both the neurology and psychology of brain injury, treatment becomes more effective, less fragmented, and far more sustainable for you and your family.
